As a client of Harmony United Psychiatric Care, you’re invited to sign up for our Patient Portal to take advantage of convenient online services.
After registering for the Patient Portal, you can:
- Schedule an appointment.
- View upcoming and past appointments.
- View and download lab results.
- Request medication refills.
- Send a question or message to your provider.
- Update your insurance, pharmacy, patient history, and other account information.
How to Register for the Patient Portal
- To get started in the Patient Portal, email [email protected] or call (800) 457-4573 to request your personal activation code.
- Once you receive your activation code, go to hupcfl.com/portal to sign up. Select the blue “Activate account” button.
- Fill in and submit the form to set up your account.
Now that your account is set up, logon anytime to access the services listed above. All services are available in the left navigation pane of your account.
